Patent is defined as the bundle of rights of an investor or his assignee to have â€Å"exclusive rights† granted by the state for a fixed given time in trade for a discovery of an invention (Freeadvice.com 2008: 1). Based from this definition of patent, it is already clear that an individual can have the freedom to solely produce a certain good given that he/she invented it. This sole production of an individual or a firm triggers for the existence of monopoly in the market. Monopoly requires only a single producer and many buyers. Therefore, with the aid of patent, an individual can easily engaged into monopoly since the government would provide that said person legal protection and privilege to be the sole producer of a certain good after a specific period of time. This legal protection of a certain invention serves as the barrier for other people to reproduce it other than the inventor or his assignee. As for the case, due to the discovery of sucralose as a substitute for sugar the state awarded Tate & Lyle its property rights over sucralose leaving them the sole producer of the said products. Since the company is the sole producer in the market of sucralose, it turned out that the company already created monopoly in the market. Other market players can only start to produce sucralose only by 2020, which means, no other market entities will be able to supply the market with the said product other than Tate & Lyle for the next 12 years. Using examples from the data, explain why firms take out patents. It was identified and stated on the given case that the production of artificial sweeteners in the market is very profitable to a point wherein almost 20 percent of the total profit of Tate & Lyle comes from their production of sucralose. Tate & Lyle is already on the process of putting more manufacturing plants to further increase their production capacity to supply the high demand of artificial sweeteners in the market which eventually to the acquisition of more profit out of their production. This high profitability in the monopoly of Tate & Lyle of sucralose made other firms in the market to eagerly find ways to enable them to also produce artificial sweeteners in the market legally. Even if the patent becomes expired by 2020, the demand of artificial sweeteners will be high enough to accommodate the entry of other producers of artificial sweeteners in the market. In addition to this, since Tate & Lyle charges high prices on their artificial sweeteners, industries that use artificial sweeteners as one of their inputs wanted to take out the patent rights of Tate & Lyle to lower down the price of artificial sweeteners in the market as competition step into the market. Although artificial sweeteners are already cheaper as compared to conventional sugar in the market, industries would still want to further lower down its prices by infusing market competition through getting rid of Tate & Lyle’s patent rights over the production of artificial sweeteners in the market. Discuss whether patents in the artificial sweeteners market lead to market efficiency or market failure. In this regard, it is clear that it is not only the consumers that will be benefited by the removal of patent rights to Tate & Lyle but also other firms that use artificial sweeteners as a factor of production as substitute to the conventional sugar in the market. Yes, it is true that the discovery of artificial sweeteners provided benefits to the market as a replacement to sugar, but that benefits can still be further improved if its prices will be controlled by market competition and not by simply monopolizing agent who’s goal is to on how to increase their profit through utilizing the bundle of rights that was given by the government. But on the other side of the coin, of the government would remove property rights to those who will discover something new in the market, there is a big possibility that they will be de-motivated to have an initiative to conduct researches and make inventions since it will be easily copied by other firms in the market thereby defeating the initial purpose of conducting research – provide ways to improved the market position of the company. Well, this scenario is inevitable since the government would still have to protect the interest of the consumers as well as the inventors. As an alternative to solve this problem of improving the quality of welfare of both parties, the government could provide patent rights to a certain company but with a condition of setting floor price or price ceiling in order to protect the interest of the consumers as well as the company itself. In recent years, China has met price of commodities rising quickly, inflation has affecting the overall economy. In this article, I will analyses last five years status of China inflation, explain the cause and effects, then base on the cause and effects, giving some solution to deal with the inflation. According to the definition of economics, inflation refers to the number of currency in circulation more than economic operation needed, and then cause currency devaluation and a persistently rising price level. In order to precisely explain inflation in China, I selected last five years inflation rate to analyze the overall condition about China inflation. Inflation rate, The following chart shows last five years inflation rate and trend of change. * In these chart, we can clearly see that, after 2008 years high inflation rate, inflation rate has fallen during the 5 years. But the size is not big. Since 2009, the inflation rate was going a new round of the rising trend. And during last thirty years, overall commodity price in china has kept rising quickly. So China inflation is still a serious problem. In order to deal with the financial crisis in 2008, to speed up the pace of economic growth in China, the central government carried out 4 trillion RMB investments and a series of stimulate measures to expand domestic demand. In China, the fixed assets investment increased 30 %( compared to 2008 )ï ¼Å'the amount of fixed assets investment is 22.46 trillion RMB in 2009( 65.9% in annual GDP ) and 27.81 trillion RMB in 2010( 69.8% in annual GDP ). Recently a few health problems have been noted to have a serious impact on the health of the inmates. Some of the common terminal conditions that can affect inmates include cancer (Kinsella, 2004). The most common infectious disease noted in prisoners is Hepatitis C, which affects about 33% of the inmates in Texas, and 41% in California. The most common mean of spreading Hepatitis C in prisons is through contaminated blood or IV drug use with contaminated needles. $24000 to $30000 is spent each year on treatment for Hepatitis B. About 10% of the inmates are mentally ill (Kaplan, 1999), however recent figures suggest the figures could be as high as 16%. There were just very few prisons that were providing mental health facilities to the prisoners. Small percent actually receive mental treatment programs and an even smaller group receives medication to treat the condition (Kinsel la, 2004). Communicable diseases may not only be a problem for inmates when they are in the prisons, but also when they are released. About 2.5 to 4.5 % of all the inmates are affected with an STD. It costs about $475 to treat the STD’s. 2.2% of the inmates have HIV infection and about 0.6% have AIDS. The prevalence of the condition is about 3 to 4 times higher than the general population. About $195000 is spending each year for the HIV management of inmates (Kinsella, 2004). Several cost-saving measures can be initiated as an effort to lower costs in prisons. These include privatization of the healthcare services, having disease prevention and health promotion programs, inmates co-sharing healthcare costs with the government (copayments), using telemedicine facilities, and early release of those inmates who are sick, terminally ill, or very old. Prisons need to develop better healthcare policies in order to handle the crisis of rising healthcare costs, using greater innovati on (Kinsella, 2004). Society of American Naturalist Essay Different organisms live in a specific place in the biosphere called their habitat. Through the course of evolution, species have adapted to their environment by growing certain physical structures or developing certain characteristics. Factors such as availability of resources (such as light intensity, water availability and others) contribute to the success of survival among species in the biosphere (Regents prep, 2003). Also, biotic relationships (such as competition, mutualism and others) interact to the dynamics of species survival. Certain species of plants grow abundantly in certain areas only of the biosphere. Some grasses grow abundantly in area where there is ample source of sunlight, but does not grow within forest beds where sunlight is scarce. In the same way, succulent herbs and vines are not commonly seen in dry area like the grassland. This exercise aims to characterize the effect of light intensity to the survival of grassland monocots. Hypothesis Limit in the availability of resources would cause decrease in species productivity and may reduce their chance of survival. For plants that are adapted to direct sunlight exposure may not be able to survive when placed in an environment where there is less amount of light. Methods For this experiment, effects of changes in lighting condition to grassland plant will be tested. To do this, find a grassland area (or any area that are exposed to direct sunlight most time of the day) and select species of low growing monocots. The area should be occupied dominantly by a single species in order to make easy observation. Measure six feet by six feet (6 x 6 ft) working area. Using small peg and nylon string mark the boundary and divide the working area into four equal quadrants. Tag the quadrants as area I, II, II and IV as shown in figure 1. I II III IV Figure 1. Using long sticks and cheesecloth (or any material that would cast greater shadow), make a tent over quadrant III and IV making sure that quadrant I and II are not shaded. The tent will simulate shaded condition and limit sunlight exposure for species in quadrant III and IV, while ensuring that other condition such as, humidity, mineral and water contents of soil and interacting species (insects and other plants) are held constant. In making the canopy, leave at least half foot (0. 5 ft) gap above the tip of the grass so that air can circulate in the area and will not cast shadow in area I and II when direction of sunlight changes. Quadrant I serve as the control while quadrant II serves as a duplicate for the control. Quadrant III is the experimental and quadrant IV is the duplicate. Make a table and list the initial condition of the selected grass species found in all quadrants. Take note of the time observation was made and the condition of the environment such as temperature of the surrounding air and the soil, humidity. Measure the difference in the amount of light in each quadrant. For two weeks, observe any changes in the condition of the grasses, such as leaf color and turgidity in all quadrants. Collect data every two days, preferably the same time when the set up was made. Predictions There will be noticeable changes in the condition of plans found in the given set up. Because these plants are not adapted in colder or shaded area, most leaves of grasses III and IV would have turned yellow and noticed decrease in their turgidity. Some leaves may have already dried out because photosynthesis is not carried out in its maximum potential. Grasses found on quadrants I and II should have remained the same condition as the start of the setup, because all factors necessary for its survival is available. Conclusions A change in the environmental condition and limit of resources affects the chance of species survival. Plants that are found in grassland area that are adapted to prolonged sunlight exposure may not be able to survive when placed in an area where there is a limited supply of sunlight. Part II Evolution of Resistant Bacteria Mutation of infectious bacteria to a resistant strain seems impossible to stop. Biological species are not fixed entities and are subject to ongoing modification through chance and adaptation (Society of American Naturalist, 1998). Organisms (such as bacteria) interact with its environment (their host and medicines) in a very dynamic way that it is biological phenomenon to adapt to these changes. Therefore, evolution of mutant genomes is inevitable. In natural environment, formation of resistant form of bacteria had been found. Antibiotics produced naturally by the soil, waste products and treatment waste are also contributes to evolution of these organisms. Since there is no way of stopping evolution, the only possible solution to this problem is to slower down its process. Though there is no possible way to quantize the evolution process or exactly measure its progress, it is the idea of determining the key point in the evolution process and tackles the issue at that point. Resistant forms of bacteria comes form strains that are not fully killed by antibiotics. There are cases when a patient was instructed to take medicine for a certain period but discontinue taking the medication when they feel that they are already well. However, they do not know that bacteria causing their sickness were just dormant or too few to cause illness. Or some resort to self medication by taking readily available over the counter antibiotics. Medical professionals on the other hand contribute to the matter. Administering prescription drug has been a normal way in treating diseases. However, because of prolonged use to these antibiotics, resistant forms of bacteria evolve. There is theoretical evidence proving application of selective pressure (such as antibiotics in food animals) will result in drug resistance by pathogens attacking human. It may not be easy to get direct evidence but this could help in development of policies (Leidberg, 1997, p. 419). Educating the public about the risk of self medication and drug resistance is a good step. Including this topic to academic lessons is also a good way in reaching peoples attention. Also, it is not just the public who should be serious about his matter. Medical professionals should also be aware of the risk involve in prescribing common antibiotics to most illness. After all, they are the ones that have direct knowledge about medicines and illnesses. The Roper Logan Tierney Model Nursing Essay The aim of this study is to demonstrate the care management that a nurse can provide to a patient who had Laparotomy and mesh repair of incarcerated incisional hernia. It also shows how the Roper- Logan- Tierney Model was utilized in assessing, planning, implementing, and evaluating patients care. The core of this care study is on infection and wound healing management which was identified as the patients main problem. According to NICE (2008), a minimum of 5 out of 100 surgical patients develop infection and that this covers almost a quarter of all infections acquired in a hospital. The patients name was altered to uphold confidentiality (NMC, 2008). In this study, health and social care policies affecting the patient care were also considered. PATIENT PROFILE Mrs P is a 63 years old lady who was admitted due to symptoms of small bowel obstruction like vomiting, bloating and abdominal pain. She lives with her husband in a privately owned house who also has impaired mobility due to stroke. Mrs P weighs 111 kilograms, 5 feet 7 inches tall and body mass index of 38. Previous medical history includes Chronic Kidney Disease Stage 2 (2008), Congestive Cardiac Failure (2007), leg cellulitis (2006), Essential Hypertension (2005), Primary repair of Incisional Hernia (1992), Type 2 Diabetes(1991), Repair of Umbilical Hernia(1985), Total Abdominal Hysterectomy NEC (1979) and Cholecystectomy (1976). After series of examination, she was found to have incarcerated incisional hernia which was repaired with mesh on the emergency list. Post- operatively, she was admitted to ITU for ventilator support and post-op care. When she was stable, she was transferred in the ward and two weeks post- op she developed infection and her abdominal wound dehisced. Patient has to take several medications while in the hospital to help her recover. She had Augmentin (Co- amoxiclav) 625 mg via oral route three times a day, followed by tazocin (Piperacillin with Tazobactam) 4.5 grams intravenously every 8 hours interval, Fragmin (Dalteparin) 7500 units once at 6 pm, senna (7.5mg) two tablets in the evening, bisoprolol 10 mg once daily, furosemide 20 mg daily orally, ramipril 5mg daily orally, amlodipine 10 mg daily oral, paracetamol 1 gram 4-6 hourly oral, domperidone 10mg three times daily orally, insulin Glargine(Lantus) twice a day SC injection, Novorapid three times daily SC injcetion, and oxycodone hydrochloride (oxynorm) 10 milligrams every 4-6 hours orally when needed. PATHOPHYSIOLOGY The development of an surgical site infection depends on contamination of the wound site at the end of a surgical procedure and specifically relates to the pathogenicity and inoculum of microorganisms present, balanced against the hosts immune response(NICE,2006). Typically, wound infection is caused by migration of patients normal flora to the wound site. Another way of surgical site being contaminated is being in contact with contaminated surgical equipments, environment, and hands of staffs. (NICE, 2006). ASSESSMENT The Roper- Logan-Tierney Model of Nursing based on activities of living was used in planning the care of Mrs P which is a widely used model in practice areas in the UK(Roper et al 2000). Maintaining a safe environment Mrs P was alert and orientated however she is diabetic, hypertensive, uses eye glasses and obese. She has an infected wound that is oozing and painful, poorly healed, dehisced, abdominal wound. She has a urinary catheter in situ which can be a potential site of another infection. Communicating Patient can communicate effectively, her primary language is English, can hear and talk without any aids and difficulties, and very conversant but when it comes to her care she tends to stay quiet and just wait for her turn to be look at. Breathing Patient is at risk of developing chest infection due to decreased mobility and respiratory depression due to oxynorm medication. Patient has a respiratory rate of 15 breaths per minute, oxygen saturation of 98% on air, no shortness of breath noted, no signs of respiratory distress and no complaints of pain during breathing. Eating and Drinking Patient claims to have decreased appetite after operation and risk of poor wound healing since although she is obese, she still needs some important nutrients like protein and vitamins like vitamin A,C, and K. Also patient is scared that when she ate, it will cause more pressure to her abdominal wound. Patient is able to eat and drink independently, no complains of difficulty of swallowing, and she is on normal diet. Eliminating Mrs. P has urinary catheter in situ draining adequate amount of urine at present which made her at risk of further infection. She was continent of faeces and uses bedside commode with assistance of two staffs. Because of the pain on her wound when moving and the need of two staffs to help her get out of bed, she keeps refusing her senna tablet and end up opening her bowel on bed. Personal Cleansing and dressing Nurses are the one irrigating and changing her wound dressing. Mrs P is unable to wash and dress herself independently due to her current state of health. Did not have any shower since admission because of her difficulty in mobilising. Controlling body temperature Although Mrs P got an infection, her temperature during assessment was 36.8 degree Celsius, no sweating noted, skin warm to touch, uses only hospital gown and dressing gown to keep her warm during the day, and uses top sheet and one blanket at night. She has limited mobility which predisposed her to poor blood circulation. Mobilising While on bed, Mrs P can turn on her sides but still with assistance of one staff because of her abdominal wound and shes an obese patient. Patient complains of pain on the surgical site when moving and mobilising. Three days before she was being hoisted from bed to chair and back to bed with assistance of 3 staffs but at this time after referral to physiotherapist, she can transfer to chair with assistance of two staffs and use of a zimmer frame. She was able to stand during the transfer and can make 2-3 steps during transfer. Working and playing Patient worked in an office before but had early retirement due to illness. Enjoys knitting and playing with her grandchildren at home. Although she can still do knitting, she cannot run or look after her grandchildren at home in her situation. Expressing sexuality Mrs P is 63 years old, menopause, and still lives with her husband. Her abdominal wound makes her anxious about her body image. Sleeping Patient says shes not able to sleep well due to environment change, pain and sometimes bowel urgencies. Mrs P takes two glasses of milk before bedtime. Death and Dying Mrs P keeps asking about worst thing that could happen to her regarding her present condition. She is worried for her husband when it happens to her first. Patient does not have a will. IDENTIFICATION OF PATIENTS PROBLEM While in the ward, Mrs P was assessed using the RLT Model based on activities of living. From those activities, all problems identified were related to her infected wound that is not healing normally. She has to stay further in the hospital until her infection is dealt with and that her condition will be manageable by primary care. This is a serious problem that if not attended immediately would cause further injury or problems to Mrs P therefore these problems which are related to each other should be the priority and the focus of her care plan. GOALS After nursing interventions, Mrs P will be able to verbalize feelings regarding her condition and understand the course of treatment being done to her. In three to 7 days, patient will be able to mobilize on her own using her zimmer frame and will be infection free. In order to maintain it, the NHS is regulated by several policies. DH policies are designed to improve on existing arrangements in health and social care, and turn political vision into actions that should benefit staff, patients and the public (DH 2010b). Mrs Ps information are compiled in a folder and kept in a secure place so that only members of the Multi-disciplinary team responsible for her care will be able to access it. It is the responsibility of healthcare professionals to safeguard their patients information and share it only to appropriate individuals (NMC, 2008). Mrs Ps personal information were handled in accordance with Data Protection Act of 1998. According to Mental Capacity Act of 2005, every adult has the right to make his or her decision and must be assumed to have capacity to make them unless it is proved otherwise. Informed consent was taken from Mrs P before any procedures or treatment was given or done. Doctors, anaesthetists and nurses has the responsibility to explain all tests and procedures being carried out on her and made sure she understood why it is being done or given to her. The consent is not valid when the person did not understand intervention (DH, 2009a). The vital signs of Mrs P were kept monitored and documented using the National Early Warning Score (NEWS)Chart. It is a new observation chart (implemented July, 2012) used in the ward where Mrs P was admitted. RCP (2012) says that this is also used as a surveillance system for all patients in hospitals, tracking their clinical condition, alerting the clinical team to any clinical deterioration and triggering a timely clinical response. Another tool used in Mrs Ps ward is the SBAR Tool. It is a structured method for communicating critical information that requires immediate attention or action(NHS Website, 2008). The patient was also assessed using the Waterlow Pressure Ulcer Risk Assessment Tool and Malnutrition Universal Screening Tool(MUST). In the chart it says there that although the later was incorporated to Waterlow, they should be assessed individually to ensure patient needs are addressed and their care was implemented (Waterlow 1985, Revised 2005). This tool helps nurses and other healthcare professionals in identifying what measures and equipment are needed for the care of the patient. NICE (2006) recommends that all hospital inpatients on admission and all outpatients at their first clinic appointment should be screened (weighed, measured and have Body Mass Index (BMI) calculated). Screening should be repeated weekly for inpatients and when there is clinical concern for outpatients. The abdominal wound of Mrs P was also assessed and documented on the Wound Management Chart implemented by the trust. Mrs Ps surgical team decided to leave her dehisced wound open and heal by second intention. The team looking after her prescribed antibiotic, and twice a day irrigation and dressing of her wound. Mrs P was referred to the Tissue Viability Nurses (TVN) for advice on appropriate dressings to be used on her wound as it was planned to heal by secondary intention (NICE, 2012). Another issue to consider in looking after Mrs P was infection control. The fact that she was already infected does not mean healthcare staffs no longer follow Infection control procedures. Infection control should be strictly manifested in order not to aggravate her situation. Hand hygiene is important especially before touching the patients wound to minimise introduction of pathogens and also after changing wound dressings to prevent self and cross- contamination of patient. The WHO (2006), provided Five Moments as to when healthcare professionals should perform hand hygiene. Use of appropriate Personal Protective Equipments (PPE) should also be worn when irrigating and renewing the surgical wound dressing of Mrs P to prevent spread of infection. Guidelines on how to put on PPE and how to take it off were also produced by WHO(2007). Proper disposal of sharps used by Mrs P like needles used for her antibiotic, insulin, and fragmin injection should also be observed. They should be th rown directly to designated sharp bins as uncapped and still assembled (RCN,2011). OVERALL EVALUATION AND CRITIQUE OF FRAMEWORK The nursing process is an orderly method of designing and providing nursing care which are collecting information and assessing the patient, planning care and defining objectives for nursing care, implementing interventions and evaluating results (Uys Habermann, 2005,p.3). Roper et al(2000,p.124) pointed out that assessment which is considered to be the first phase of the nursing process should be done regularly and not only once. Being this the case, the use of Roper- Logan-Tierney (RLT) Model based on Activities of Living is preferred for Mrs Ps case. It does assess the patient needs wholly and can determine the impact of infection and poor wound healing to her identified twelve activities of living as recognized by the model. Using RLT model, an overview of Mrs Ps health status was created and from it prioritization of her needs is easier. This model enabled healthcare professionals to produce a care plan which is unique or aligned to her needs. The author of this care study find this framework to be an ideal model in assessing patient and useful for healthcare professionals as they do not miss out any aspect of care. Looking at the down side of the model, it is a very long process of assessment and it takes time for a healthcare staff to finish all the twelve areas. The author of this study thinks although it is an ideal one that in a ward where Mrs P was confined, there might be an issue in doing this regularly to all patients. It is a very busy ward and if all patients are assessed regularly using this model, problems on other aspect of nursing process will be left untouched and cause additional work to incoming staffs. CONCLUSIONS AND IMPLICATIONS FOR FUTURE PRACTICE Health and social care policies are indeed very important and has a great impact to the healthcare system. It serves as the basis for the trusts in making their own policies and it guides healthcare professionals to their practice. Nurses practice in accordance with the NMC Code, Standards of conduct, performance and ethics for nurses and midwives and other laws and guidelines provided by the British government and different departments or agencies like the Department of Health, National Institute of Clinical Excellence, World Health Organization, etc. After using the Roper-Logan-Tierney Model The care of Mrs P went smoothly during her stay in the hospital. Her post- operative complications have been managed without any major issues. Since the very start, nurses and other members of the MDT reassured her that personal details and all information regarding her care is treated as confidential and that this could only be shared to appropriate people only on her approval. The MDT members based their interventions on the protocol and policies of the trusts which was based on National policies. Mrs Ps problems were managed by the interventions provided in the hospital and was now discharged and back to her home. Long term goals were also taken into account therefore a proper referral to the district nurse was done before she went home.

All they basically wanted was security and to be a happy family. It’s not so easy in today’s modern world where an employee has so many more options. I will discuss how we manage to keep employees loyal and satisfied in their jobs at our company. The role of the manager is very wide, expansive and covers several different areas all at once. Our managers act as something similar to a press secretary in that they escort visitors and play tour manager of course, they also handle all the human relations tasks such as hiring, firing and motivating the employees under their charge. So far as relational roles they also act as liaison due to the fact that much of the information they use they acquire from the employees. Our manager’s act in several other roles with the information they receive. As a monitor in that they attempt to predict what to expect of competitors. Secondly, as a disseminator because much information has to be relayed to employees either by the manager or through some sort of media. Thirdly, with visitors they tend to act as spokesperson, also. Managers have to make many decisions in our organization because we try to avoid micromanagement, Due to the avoidance of that micromanagement the manager tends to act as entrepreneur sometimes because they are making decisions that will affect the organization as a whole. Secondly, they act as police somewhat in that they need to settle disputes between employees. They handle the acquisition of supplies, also, whether it is personnel, physical supplies or monetary resources. Lastly, our managers also need to act as negotiators because they may need to sacrifice something important out of their own team to gain an important piece of equipment from another team. The organization plays a very large role in motivation and morale employees need to know that when they come to work everyday they won’t have several different tasks thrown at them throughout the day. This reflects totally as to how the individual reacts towards the organization. We combat this â€Å" Hey, you!† effect in our organization with a couple of methods.

Nelson Paul Poynter was the owner of the St. Petersburg Times, in Florida, which for years has enjoyed the reputation as one of the best newspapers in the United States.
